I authorize BreastfeedLA to photograph/video myself and/or my child (children) during this event. I hereby acknowledge that these photographs/videos may be used for the purpose of education and the promotion of breast/chestfeeding. This authority specifically includes incorporating these photographs, videos, or replicas thereof in any material, including a website or online course. I acknowledge that these images will not be used for any commercial purpose.
I understand that courses are NON-REFUNDABLE. Credit for future trainings is available under special circumstances until two weeks prior to the event. In the case of a missed class session, BreastfeedLA allows for one (1) full day make-up session in a future course offering to each student free of charge (verified personal medical circumstances as an exception). Any additional sessions needed will be prorated based on course price.
